The weekend staffing gap in Texas nursing homes
Care does not pause on Saturday, but skilled staffing does. Using CMS payroll-based data (2026-09-05) for 1,119 rated Texas nursing homes, we compared weekday and weekend nurse coverage. Registered-nurse care falls about 24% on weekends — from 0.42 to 0.32 hours per resident per day — while total nurse staffing drops only about 12%. The most skilled care is cut nearly 2.0 times as hard, at the time outside oversight is lowest.
The gap is not confined to struggling facilities. The weekend RN cut at top-rated (4-5 star) Texas homes (22%) essentially matches the cut at the lowest-rated ones (21%). And 83 homes (7%) run weekends at under half their weekday RN rate. These homes show the widest gaps:

Why the skilled cut is the one that matters
Total nurse staffing barely moves on weekends because nurse aides — who handle bathing, feeding, and mobility — stay relatively constant. What thins out is the registered-nurse layer: the license level that catches a worsening infection, adjusts medications, and decides when to send someone to the hospital. That is why RN care falling 24% while total staffing falls only 12% is the finding to watch. The clinical judgment in the building drops far faster than the raw headcount suggests.
It also cuts across quality tiers. A five-star overall rating reflects a home's average record, not its weekend coverage, so families who screen on the star rating alone can still land in a home that runs thin on Saturdays and Sundays. The rating is a starting signal; the weekly staffing pattern is a separate question worth asking directly.
How we built this
We took every rated Texas nursing home in CMS Care Compare (Nursing Homes), dataset 4pq5-n9py (retrieved 2026-09-05) with at least 25 residents and a real registered-nurse staffing figure. The headline RN comparison uses CMS reported RN hours per resident per day, weekend versus overall. The total-staffing comparison uses case-mix-adjusted total nurse hours, weekend versus overall, so each contrast is measured on one consistent basis. A home with no valid figure is excluded, the same publish gate used across the site. A statewide average can hide a home that keeps steady weekend coverage, and the reverse, so always check the individual report card. Full details on our methodology page.

Do nursing homes have less staff on weekends?
Yes. Across 1,119 rated Texas nursing homes in CMS data (retrieved 2026-09-05), registered-nurse staffing falls from about 0.42 hours per resident per day overall to 0.32 on weekends — a cut of roughly 24%. Total nurse staffing (which includes aides and LPNs) drops only about 12%, so the reduction lands hardest on the most skilled care.
Why does weekend RN staffing matter?
Registered nurses are the license level that assesses changing conditions, manages medications, and escalates emergencies. When RN hours thin out on weekends, the residents most likely to deteriorate have the least skilled coverage at the exact time family visits and outside oversight are lowest. Researchers call the resulting rise in adverse outcomes the "weekend effect."
Do only low-rated nursing homes cut weekend staffing?
No — and that is the most striking part. The weekend RN cut is about 21% at 1-2 star Texas homes and about 22% at 4-5 star homes. It is a near-universal industry pattern, not a marker of a bad facility. A high overall CMS rating does not mean a home keeps skilled staffing steady across the week.
How was this measured?
We used CMS Care Compare payroll-based staffing data (retrieved 2026-09-05) for every rated Texas home with at least 25 residents and a real RN figure. The RN comparison uses CMS reported RN hours per resident per day, weekend versus overall; the total-staffing comparison uses case-mix-adjusted totals, weekend versus overall, so each contrast sits on one consistent basis. A "severe" gap means a home's weekend RN hours are under half its overall rate.

Every Texas nursing home report card shows its staffing hours, ratings, fines, and inspection record in plain English — and a good question for any tour is simply how many registered nurses are on the floor on a Sunday.
