From CMS Care Compare (Nursing Homes), dataset 4pq5-n9py (retrieved 2026-08-09), we kept only homes with a five-star CMS overall rating, no federal abuse-warning flag, no immediate-jeopardy inspection findings, and at least 25 residents so the staffing math is stable.
